Your Choice Savings Program will include discounts on vision care. Outlook vision care has average discounts from 10% to 50% on frames, contact lenses, prescription lenses, and sunglasses. Participants have access to local and nationwide Optometrist and Ophthalmologist. Participating providers include major chains and independent vision care facilities. 85% of the U.S. population resides within a 15-minute drive of an Outlook provider. There are no limits on how many times can use your vision care membership. Simply show your membership card at a participating provider and receive savings. Saving include but are not limited to:

There are over 55,000 pharmaceutical locations that participate in the discount program. Your membership card will give you savings up to 25% off brand name prescription drugs and up to 40% savings on generic drugs. Individuals and families can use their prescription membership as many times as they need to. Just show your membership card at participating pharmaceutical location. In order for the savings to apply, Membership card must be presented and prescriptions must be paid at the time they are delivered.
Members also have access to prescription mail order services. Savings up to 40% is also available through mail order services. This service is very convenient for members with maintenance medications.

After joining the Choice Savings program, you will receive a membership card. Simply show your membership card to a participating provider (access dental providers above) at each visit and receive pre-negotiated discounts. There are no claim forms to fill out. Discounts are received at the time of service. Members may visit any participating provider on the program list. Members can change providers at anytime. You can use your membership card as often as you would like.
The Choice Savings Program will save its members 15% to 50% on dental procedures per visit. Choice Savings Program members have the ability to save significantly with their dental plan because members have the freedom to use their plan over and over with no limits. Members of the Choice Savings Program will save 15% to 50% per visit on cosmetic dentistry (including porcelain veneers, orthodontic treatment, teeth whiting, and more). Typically the savings received in one visit to the dentist will pay for the membership itself.
